Unfortunately, we are unable to take on any new Medicaid, Apple Care or Medicare patients.

Because none of our providers participate in Medicare, when patients reach the age at which they must access their Medicare benefits, our providers will not be able to continue providing care for them. This applies to Medicare Advantage plans as well.

We will do our best to provide suggestions on where to establish for culturally competent, queer-affirming health care.

Gotchas

Although you may find your insurance company listed, please keep in mind that these companies offer many types of coverage plans. Sometimes these plans are more restrictive in which providers you can see (local examples include Virginia Mason Premera and UW Premera plans). Please contact your insurance plan to confirm that we are in-network before establishing care.

Another option…

Direct Primary Care (DPC) is a new model of healthcare delivery. Unlike traditional practices, DPC providers do not bill your insurance. Instead, patients pay a monthly fee for their care at CHM. In this case, providers are not beholden to insurance rules and requirements, and patients are not subject to surprise bills from copays, coinsurance, and deductibles. In removing this interference from insurance companies, we are freed to provide affordable excellent care.

Virtual vs. in-person visits

The SARS-CoV-2 pandemic has changed medicine forever. Our clinic is now about 40% telemedicine-based. In-person visits can be coordinated between an established patient and their provider or a staff member, but must be specifically discussed/requested.

For patients who have already booked their initial encounter, we ask that you complete the following tasks ahead of your first visit:

  1. Complete the four online questionnaires (New Patient Questionnaire, HIPAA Disclosure, Financial Responsibility Agreement, and Release of Information). A link to these forms will be sent to you by the staff at the time of booking, and again 48 hours before your visit.

  2. Please take one photo each of the following:

    Front of insurance card

    Back of insurance card

    Front of ID/Drivers License

  3. Upload these insurance/ID photos into your Elation Passport account*.

    *This assumes you’ve already set up an account with the patient side of our electronic medical records system, called Elation Passport. If you’ve had trouble setting up an account, please contact the front staff for assistance at 206-720-9999 before completing these tasks.
